PF is contributed every month by both the employer and the employee. An employee can only receive the contribution made towards PF during his employment, but there are some exceptions.

Form 2

Form 2 is filed in declaration and enrollment under the flagship scheme of Employment Provident Fund and Employment Family Pension Scheme. Form 2 should be filed by the employees who are joining the establishment. This form has to be submitted along with Form 5. Form 2 is divided into 2 separate parts: 

Part A: Part A of Form 2 deals with designating the recipients of the EPF balance in the event of the death of the particular account holder. This Part of the form should include the following details:

  1. Name
  2. Age
  3. Address
  4. relationship with customer
  5. Guardian’s information (if the nominee is a minor)
  6. Sum of the amount to be paid to the nominee

This part needs to be signed, or a thumb impression needs to be put at the end of the section.

Part B: As already mentioned in Part A, the nominee statement should also be included in Part B. In addition, the details of members eligible to receive children/widow pension should be furnished. 

Again, this part should be duly signed, or a thumb impression should be put at the end of the section. ,

Form 12A

This Form 12A is a report containing the payment details contributed to the employee’s account concerned in a particular month.

Annual PF Return Filing

Annual return is to be filed by 30th April in a given year. The forms used for filing annual PF returns are:

  1. Form 3A
  2. Form 6A

 

Form 3A: Form 3A shows the month-wise contribution of subscribers or members and employers towards Employees’ Provident Fund and Employees’ Pension Fund in a year. The data is calculated by each member who is part of the plan. In addition, the system will include the following details:

  1. Subscriber Name
  2. Account Number
  3. Father’s or Husband’s Name
  4. Establishment Name and Address
  5. The statutory Rate of Contribution
  6. Voluntary Contribution, If any.
  7. Form 3A Must be Signed and Stamped by the Employer.

 

Form 6A: Form 6A is a compact annual contribution statement containing details about the annual contribution of each member of the establishment. The Form has to include the facts as they are given below:

  1. Account Number
  2. Subscriber members name
  3. If any, wages, retention allowance, and DA include the cash value of food concession paid during the currency period.
  4. The amount of contribution which is deducted from wages.
  5. Employer’s Contribution (Both EPF and Pension)
  6. refund of advances
  7. Higher rate of voluntary contribution (if any)
  8. Comments

 

In addition, the following details should also be included in the amount Remitted column:

  1. Month of Contribution  
  2. Remittance contribution which provides for refund of advances
  3. EDLI Contribution
  4. pension Fund Contribution
  5. Administrative Fee
  6. Total Contributors.

Form 5

Form 5 is a monthly report containing the details of the employees newly enrolled in the provident fund scheme. Form 5 should include the following information:

  1. Name of organization
  2. Organization Address
  3. date of joining
  4. Work track record.
  5. organization code
  6. name of employee
  7. employee account number
  8. employee’s date of birth
  9. Middle Name (Husband/Father)

The form must be filed by the employer and stamped with the date of filing mentioned on it.

Form 10

It is a monthly report which contains the details of the employees who have not become part of the scheme in the given month. Form 10 contains the following information:

  1. Name of Employee.
  2. Account Number.
  3. Father’s or Husband’s Name.
  4. Date of Leaving Service.
  5. Reason for Leaving the Service.

Form 10 should be filed and stamped by the employer with the date of filling the form.

Annual Account Statement

The employer has to file the PF return through the form mentioned above, the Employees Provident Organization dispatches the annual account statement of each subscriber through the employer. The consideration of accounts shall reflect the following prevailing particulars:

  1. The opening balance of the subsidy includes the interest of both the employer and the employee.
  2. The annual contribution of both employer and employee.
  3. interest earned on the contribution
  4. Employers and employees give total grants.
